🏅 Student Sports Physical Form

All students who wish to participate in a school sport must complete a Physical Form.

• Requirement: Students must submit a completed form before beginning any sporting activity.

• How to Obtain the Form:

  • Download: The Physical Form is available for download from this page.

  • In-Person: You may also pick up a copy from the front office.

Please ensure the form is completed and returned on time so your child can safely participate in school sports.
